LIFE (Living Independence for the Elderly) is dedicated to helping older adults maintain their independence at home while receiving the highest level of comprehensive care. Through an innovative model of care coordination, LIFE provides a full spectrum of health and wellness services tailored to each participant’s unique needs.

Cost

What you pay for LIFE depends on your financial situation. Find out more ways eligible seniors can cover the cost of LIFE below.

If you are eligible for both Medicare and Medicaid, there is no cost for any drug, service or care if approved by your LIFE health team.

If you have Medicaid, you will not pay a monthly premium for the long term care portion of the LIFE benefit

If you have Medicare but do not qualify for Medicaid, you’ll be responsible to pay for a monthly premium to cover the long-term care portion of the LIFE benefit and a premium for Medicare Part D drugs.

If you are not eligible for Medicare or Medicaid, you can pay privately.

LIFE enrollees must agree to receive all needed health care, including primary care and specialty physician services (other than emergency services), from LIFE and its network of approved providers. Participants may be fully and personally liable for the costs of unauthorized services or those out of LIFE provider network.

Eligibility

To be eligible for LIFE, a participant must:

  • Be at least 55 years old.
  • Live in the service area of one of our programs.
  • Need nursing home level care.
  • Be able to live safely in the community with help from LIFE.

Enrollment

Joining the LIFE program is a four step process. This includes an Intake, Assessment, Determination of Medical and Financial Eligibility and Enrollment. Below, you can evaluate each step more in depth.
