Understanding LIFE

Living Independence For the Elderly (LIFE) is a managed care program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in the community through supportive services.

The LIFE centers provide a single hub for primary care, pharmacy, nursing, restorative therapy, adult day services, care coordination and social work.  The center staff coordinates additional supportive services such as home health care, transportation, home accessibility modification and home delivered meals. LIFE coordinates and provides all needed preventative, primary, acute and long-term care services.  The ability to coordinate the care of each participant enrolled in a LIFE program is key to the model.

To be eligible to enroll as a LIFE participant, an individual must be:

  • At least 55 years of age or older.
  • Eligible for Medicare and/or Medicaid or have the ability to private pay.
  • Reside in LIFE’s designated service area.
  • Be assessed by the authorized state agency as nursing home eligible.
  • Be able to live safely at home with the help of the LIFE program at the time of enrollment.
