The LIFE program allows eligible seniors to live safely in their homes with access to the medical, social and rehabilitation services they need. All services provided, or arranged, by LIFE are fully covered when approved by our health team.

Eligibility & Enrollment
Joining LIFE is a four step process. This includes an Intake, Assessment, Determination of Medical and Financial Eligibility and Enrollment. Here, you can evaluate each step more in depth.
What is the cost? Good news! If you are eligible for Medicare and Medicaid, there is no cost for any drug, service or care if approved by your LIFE team. Don’t have this type of coverage? No problem. There are several other ways eligible seniors can cover the cost.
LIFE enrollees must agree to receive all needed health care, including primary care and specialty physician services, from LIFE and its providers. In addition, participants may be personally liable for any costs of unauthorized services outside of LIFE’s original agreement.
